• Ever change your name through marriage? If so, maiden name? Date?

• Place of birth
• Race
• Mother’s maiden name?
• Nursing School attended? Location?
• Date of graduation?
• Type of degree? (LPN?, ADN? BSN? etc…)
• Original State of licensure?
• Any other state licenses?
• Any disciplinary actions ever been taken against your license?
  VERY IMPORTANT! Ever been arrested? DUIs? Note: Any arrest WILL show up on your FBI background check, even if charges were dropped, dismissed or even "expunged".

Note: Fees will be as follows:

LPN/RN

$222 – application fee paid to the Florida Board
$125 – fee for the services of Florida Nurse Licensing Service

ARNP/CRNA

$322 – application fee paid to the Florida Board
$225 – fee for the services of Florida Nurse Licensing Service

Fees can be paid with a check, money order, credit card or any combination of the 3.
Regardless of which payment method you choose, we will require a credit card to secure payment for our services.
